Here are things to know to make your scheduling easier:
Before you call you may go to More, patient forms, download the form, and bring the completed form at your appointment.
Things to tell us:
1. Full Name
2. Address and phone #'s
3. birthdate
4. Both parents' names of the child
5. insurance plans name and policy #
6. if you wish a contact lens exam also
Things to take with you
1. prior glasses (even if broken)
2. insurance cards
3. list of medications
4. prior eyeglasses/ contact lens prescriptions
5. name of prior eye doctor
Our Policies
1. If you are an adult patient, do not bring any other members of the family (child or sibling).
2. If your child is the patient If a child, do not bring any other members of the family (child or sibling). Only one parent escorts the child. All of our patients, if desired, are given a description of the conditions Dr. Henshaw discussed. The parent stays with the child in each room visited (not the reception room).
3. An interpreter (only one) may accompany one who does not speak English
4. You may record your visit, but the dr. will not engage in a phone conversation during the visit.
5. Warn us if a wheelchair is used so we can prepare
