One block North of Lodi Memorial Hospital's Fairmont AV entrance at the corner of   Fairmont AV & Sylvia DR

Don't confuse Ham LN with Fairmont AV. The Hospital's main entrance is on Ham LN. We are one street East on Fairmont AV.

For your convenience, Monday through Wednesday, we are open during the noon hour; but the doctor does not examine then. On Thursday we close from 11 AM to 2 PM for our weekly staff training.
On Fridays, our schedule is dedicated to vision therapy only.
